Community Outreach

We understand that not everyone is ready for treatment right away, and that is okay. Outreach is about building trust over time. Our teams show up consistently, offer help without conditions, and let people take steps toward better health at their own pace. Sometimes that means handing someone a supply kit. Other times it means helping them get into housing or start treatment for the first time.

Our Street Outreach and Harm Reduction program provides practical supplies and resources to help people stay safe. The team distributes harm reduction kits, offers referrals to medical care, housing, and food assistance, and helps connect individuals with free HIV and HCV testing and treatment. They also lead community clean-up efforts, collecting used needles and helping keep neighborhoods safe for everyone.

The team goes a step further to ensure that people are being met where they are and not bound by systems. Our Street Team added a component to take health care even further by bringing a medical team directly into the community. A Nurse Practitioner, Community Health Worker, and harm reduction specialist visit encampments and community agencies to provide medical assessments, wound care, treatment for infections, and follow-up support. Street Medicine team is ensuring real health care is delivered where it is needed most, not behind the walls of a clinic, but out in the community where people live.
