How a Medical Case Manager Helps You

Your medical case manager starts by learning about you. They sit down with you to understand your health needs, your goals, and any challenges you may face. Together, you build a personalized care plan that fits your life.

Here is what your medical case manager can do for you:

• Assess your needs — Your case manager reviews your health, medications, housing, and other factors that affect your well-being.
• Develop a care plan — Together, you create a step-by-step plan to meet your health goals.
• Coordinate with your providers — Your case manager works directly with your doctors, specialists, and other health care providers to make sure everyone is on the same page.
• Address barriers to care — If something is keeping you from getting the care you need — like transportation, cost, or scheduling — your case manager helps find solutions.
• Complete referrals — Need to see a specialist or access another service? Your case manager connects you with the right resources.
• Improve health outcomes — The goal is simple: help you stay healthy and feel your best.

Medical Case Management is available to individuals who are clients of Health Plus Indiana. There is no cost to you for this service. We serve clients at all of our locations.
