The South Bend office is the central hub of Health Plus Indiana and the foundation of our operations. As our largest location, it is designed to provide a comprehensive, one-stop setting where individuals can access the full spectrum of services we offer in a convenient and coordinated way.

At this site, patients have access to high-quality primary medical care, including preventive services, chronic disease management, and routine check-ups. In addition to physical health services, we offer integrated mental health support, ensuring that individuals can receive counseling, behavioral health services, and referrals in a compassionate and confidential environment.

HIV, Hepatitis C & STI Testing

Health Plus Indiana offers free and confidential testing for Human Immunodeficiency Virus (HIV), Hepatitis C, sexually transmitted infections, and pregnancy at all three locations. No appointment is needed and walk-ins are always welcome. Get rapid HIV results in under one minute.

PrEP — Preventing HIV Before It Starts

Pre-Exposure Prophylaxis (PrEP) is a daily medicine you can take to help prevent HIV. Health Plus Indiana offers free PrEP services through our virtual clinic, including lab work, prescriptions, and regular follow-up care. A dedicated PrEP coordinator guides you through every step of the process.

Medical Case Management

Medical case management at Health Plus Indiana helps people living with HIV stay on track with their health. Your case manager creates a personalized care plan, coordinates with your doctors, and helps remove barriers so you can focus on feeling your best.

Linguistic & Translation Services

Health Plus Indiana offers translation and interpretation services in over fifty languages so that every client can communicate comfortably in the language they prefer. Bilingual case managers are available on-site to provide Spanish and English support at our northern offices.

HEP Connect — Hepatitis C Testing & Treatment

HEP Connect helps people get tested and treated for Hepatitis C, a virus that affects the liver. Health Plus Indiana provides a dedicated case manager, insurance navigation, transportation assistance, and ongoing support throughout your treatment journey. With modern medicine, Hepatitis C can be cured in most cases.

Peer Recovery Program

The peer recovery program at Health Plus Indiana connects you with people who truly understand what you are going through. Peer specialists have their own lived experience of recovery and are able to provide guidance, encouragement, and practical support to help you build a healthier life.

Primary Medical Care

Health Plus Indiana provides comprehensive medical care at our clinics in South Bend and Austin. Our providers specialize in HIV, Hepatitis C, LGBTQ+ health, and primary care. We accept most insurance plans and offer a sliding fee scale for uninsured patients.

Prevention Education & Awareness

Health Plus Indiana provides free educational workshops and presentations on HIV prevention, healthy behavior choices, and reducing risk of infection. We bring these programs directly to local businesses, schools, community organizations, and social service agencies throughout northern and southern Indiana.

Mental Health Services

Health Plus Indiana offers mental health services to support your emotional well-being and help you through difficult times. Licensed Social Workers provide both individual and group counseling sessions to help you manage stress, build coping skills, and connect with community resources.

Non-Medical Case Management

Non-Medical Case Management at Health Plus Indiana helps you navigate everyday challenges beyond the doctor's office. Our caring team assists with budgeting, legal resources, housing referrals, substance use support, and other social services to help you build a stable and healthy life.

Street Outreach & Harm Reduction

Health Plus Indiana's street outreach and harm reduction team works directly with people who are experiencing homelessness or at higher risk of infection. We provide harm reduction supplies, referrals to needed services, and connections to free HIV and Hepatitis C testing and treatment. Our Street Outreach Team also includes an NP who is able to provide care to those who are reluctant to seek in traditional facilities.

Substance Abuse Support Program

The Special Populations Support Program at Health Plus Indiana provides individual and group support for people living with HIV who are also managing substance use or mental health challenges. We offer encouragement and assistance to meet you wherever you are in your recovery journey.

Street Medicine

Health Plus Indiana's street medicine team brings healthcare directly to people experiencing homelessness in our community. A Nurse Practitioner, Community Health Worker, and harm reduction specialist visit encampments and community agencies to provide medical assessments, treatment, and follow-up support where it is needed most.

Housing & Housing Support Programs

Health Plus Indiana provides housing support for people living with HIV, including emergency shelter, short-term rental help, and long-term housing assistance. Our programs help you find and keep a safe, stable place to live while you focus on your health.

Insurance Enrollment Services

Health Plus Indiana has Insurance Enrollment Specialists at every location who are ready to help you find and apply for health coverage at no cost. We assist with Medicaid, Medicare, marketplace insurance plans, and patient assistance programs to make sure you have the coverage you need.

Food & Nutrition Program

Health Plus Indiana's food and nutrition program gives clients monthly access to a food pantry, medically tailored meals, vitamins, and nutritional supplements. Community tower gardens also provide fresh produce to help you eat well and stay healthy. Available at South Bend and Austin.

Transportation Services

Health Plus Indiana provides free transportation to medical appointments and social service visits through our Retention in Care coordinators. We also deliver food and medicine directly to clients who need it. This service is available at all three of our office locations.

Direct Emergency Financial Assistance (DEFA)

Direct Emergency Financial Assistance (DEFA) from Health Plus Indiana provides help with rent, utilities, medical costs, and other urgent expenses. This program is available to clients who face financial hardship.
