With two facilities located in Austin, we are committed to reducing barriers and making it easier for community members to access both healthcare and essential support services close to home.

This location serves as a hub for our support and outreach programs, focusing on the social and community-based services that play a vital role in overall health and well-being. Here, clients can access comprehensive case management services, judgement-free peer recovery services, and access to food assistance programs.

Beyond these core services, our team works closely with community partners to offer referrals, health education, outreach initiatives, and other supportive programs designed to empower individuals and strengthen the community. At every step, Health Plus Indiana is committed to creating a welcoming, respectful environment where everyone feels supported, heard, and connected to the resources they need.

Street Outreach & Harm Reduction

Health Plus Indiana's street outreach and harm reduction team works directly with people who are experiencing homelessness or at higher risk of infection. We provide harm reduction supplies, referrals to needed services, and connections to free HIV and Hepatitis C testing and treatment. Our Street Outreach Team also includes an NP who is able to provide care to those who are reluctant to seek in traditional facilities.

Health Plus Indiana has Insurance Enrollment Specialists at every location who are ready to help you find and apply for health coverage at no cost. We assist with Medicaid, Medicare, marketplace insurance plans, and patient assistance programs to make sure you have the coverage you need.

Health Plus Indiana's food and nutrition program gives clients monthly access to a food pantry, medically tailored meals, vitamins, and nutritional supplements. Community tower gardens also provide fresh produce to help you eat well and stay healthy. Available at South Bend and Austin.

Health Plus Indiana provides free transportation to medical appointments and social service visits through our Retention in Care coordinators. We also deliver food and medicine directly to clients who need it. This service is available at all three of our office locations.
