Health Plus Indiana provides transportation to medical and social service appointments for our clients. Our Retention in Care Coordinators also deliver food and medicine to help you stay healthy.

Getting You Where You Need to Go

Getting to a doctor’s appointment or picking up your medication should not be a struggle. But for many people, transportation is one of the biggest barriers to staying healthy. If you do not have a car, live far from a bus route, or simply cannot afford a ride, it can be hard to keep up with your care.

At Health Plus Indiana, we do not want transportation to stand between you and your health. That is why we offer transportation services to our clients through our Retention in Care (RIC) Coordinators.

What Our Transportation Services Include

Our Retention in Care (RIC) Coordinators provide the following transportation services:

• Rides to medical appointments — Whether it is a regular check-up, a specialist visit, or a lab appointment, we can take you there and bring you back.
• Rides to social service appointments — Need to get to a benefits office, a housing agency, or another social service? We can help with that too.
• Food delivery — If you cannot get to our food pantry, our team can deliver food directly to you.
• Medicine delivery — Picking up your medications is essential to your health. If you cannot get to where you need to be, we can bring your medications to your door.

Why Transportation Matters

Missing a doctor’s appointment can set back your health. When you miss an appointment, you may not get the care or medication adjustments you need. Over time, missed appointments can lead to bigger health problems.

Our transportation service helps you stay on track. When you know you have a ride, it is easier to keep your appointments and follow your care plan. Our RIC Coordinators are friendly and supportive. They know that getting to your appointment is just as important as the appointment itself.

• South Bend
• Austin
• Elkhart

How to Get Started

To schedule a ride or request a delivery, contact your nearest Health Plus Indiana office. Please call ahead of time to schedule your ride so we can plan the best route for you. We are here to make sure nothing stops you from getting the care you deserve.
