Health Plus Indiana’s street medicine team brings healthcare directly to people experiencing homelessness in our community. A Nurse Practitioner, Community Health Worker, and harm reduction specialist visit encampments and community agencies to provide medical assessments, treatment, and follow-up support where it is needed most.

Street Outreach & Harm Reduction

Health Plus Indiana’s street outreach and harm reduction team works directly with people who are experiencing homelessness or at higher risk of infection. We provide harm reduction supplies, referrals to needed services, and connections to free HIV and Hepatitis C testing and treatment. Our Street Outreach Team also includes an NP who is able to provide care to those who are reluctant to seek in traditional facilities.
