At Health Plus Indiana, we believe everyone deserves quality healthcare. That is why we work hard to keep costs low. We never want money to stand in the way of your health. Our team is here to help you find a plan that fits your budget.

Our Sliding Fee Scale

We use a sliding fee scale to set your costs. This means the amount you pay is based on your income and family size. We compare your household income to the Federal Poverty Level (FPL). The FPL is a number set by the government each year. It helps programs like ours figure out how much help you need.

Here is how our fee scale works:

• 0 to 100% FPL — Free care (100% discount)
• 101 to 150% FPL — 90% discount
• 151 to 200% FPL — 80% discount
• 201 to 250% FPL — 70% discount
• 250 to 300% FPL — 60% discount
• 300 to 350% FPL — 50% discount

Even if you do not qualify for a full discount, you may still save a lot on your care.

What You Need to Apply

To find out your discount level, please bring these items to your visit:

• Proof of income — such as a pay stub, tax return, or benefits letter
• Photo ID for all household members — such as a driver’s license, state ID, or passport
• Proof of residency — such as a utility bill, lease agreement, or bank statement with your address

You can fill out an application at the front desk of any Health Plus Indiana location. You can also apply online. Our staff will walk you through every step if you need help.

How Often Do I Need to Reapply?

Your fee scale level is reviewed once a year. If your income or family size changes during the year, please let us know. We can update your discount at any time.

Payment Plans

If you have a balance, we offer payment plans to help you pay over time. Talk to our front desk team to set up a plan that works for you. We want to make sure cost is never a barrier to getting the care you need.

Free Services

Some of our services are completely free, no matter what your income is. These include:

• Human Immunodeficiency Virus (HIV) testing
• PrEP — a medicine that helps prevent HIV

You do not need to apply for the fee scale to use these free services. Just walk in or call to set up a time.
