Our approach begins with medical care you can trust. Our experienced providers specialize in HIV treatment and take the time to understand your individual needs, lifestyle, and goals. Together, we develop a personalized care plan that works for you. With consistent treatment, many individuals are able to reach and maintain an undetectable viral load. This not only protects your own health but also means you cannot pass HIV to others through sex—often referred to as “Undetectable = Untransmittable” (U=U).
Beyond medical care, we know that managing HIV involves more than just medication. That’s why each patient is supported by a dedicated team who helps coordinate all aspects of your care. Your case manager works closely with your healthcare providers, assists with scheduling appointments, helps you understand your treatment, and connects you to additional resources. They also help address barriers such as transportation, insurance, medication access, and other challenges that might make it harder to stay engaged in care.
We also offer a wide range of supportive services designed to care for the whole person. This includes access to mental health services, peer support programs, assistance with housing and food resources, and help navigating insurance and financial programs. Our goal is to make sure you feel supported not only medically, but emotionally and socially as well.

Our non-medical case managers help with budgeting, legal resources, housing referrals, substance use support, and other everyday challenges. Licensed social workers provide individual and group counseling to help you manage stress, build coping skills, and connect with others who understand what you are going through. Our peer recovery specialists bring lived experience and encouragement to help you navigate your journey.
Practical needs matter too. Our housing programs offer emergency shelter, short-term rental assistance, and long-term housing support so you always have a safe place to live. Our food and nutrition program provides monthly access to a food pantry, medically tailored meals, and fresh produce. Insurance enrollment specialists at every location help you find and apply for coverage at no charge. And our Direct Emergency Financial Assistance program provides up to five hundred dollars per year to help with rent, utilities, and other urgent expenses.
Our Retention in Care coordinators provide free rides to medical visits, deliver your medications and bring pantry directly to your home when you need it.
