Health Plus Indiana offers emergency, transitional, and permanent housing programs for individuals living with Human Immunodeficiency Virus (HIV). Safe, stable housing is a key part of staying healthy.

Why Housing Matters for Your Health

When you do not have a safe place to live, everything else becomes harder. It is harder to take your medications on time. It is harder to get to your doctor appointments. It is harder to stay healthy. That is why Health Plus Indiana works hard to help our clients find and keep stable housing.

We run several housing programs designed to meet different needs. Whether you need help right away or need long-term support, we have a program that can help.

Our Housing Programs

Emergency Shelter (ESG)

The Emergency Solutions Grant (ESG) program provides emergency shelter and rapid rehousing funds. If you have lost your home or are about to lose it, this program can help you get back on your feet quickly. Emergency shelter gives you a safe place to stay while we work together on a longer-term plan. Rapid rehousing helps you move into your own place as soon as possible.

Permanent Housing (COC / Continuum of Care)

The Continuum of Care (COC) program offers permanent housing for clients who need ongoing support. Once you are placed in this program, you can stay as long as you continue to qualify. This gives you the stability you need to focus on your health and your goals without worrying about where you will sleep tonight.

Housing Opportunities for People With AIDS (HOPWA)

The Housing Opportunities for People With AIDS (HOPWA) program provides three types of housing help:

• Short-Term Rent, Mortgage, and Utility Assistance (STRMU) — If you are behind on rent, mortgage, or utility payments, STRMU can help you catch up. This is short-term help designed to keep you in your current home.
• Tenant-Based Rental Assistance (TBRA) — TBRA provides ongoing rental assistance for up to two years. This program helps you afford a safe place to live while you work toward greater stability.
• Operating Costs (OPS) — OPS funds help cover the costs of running housing units so they remain available for clients who need them.

Transitional Housing

Our Transitional Housing program is funded through the Indiana Department of Health (IDOH). It provides housing assistance for up to six months. During this time, you work closely with your case manager to build skills and find permanent housing. This program serves as a bridge between emergency shelter and a place of your own.
