We know that many obstacles can stand in the way of your health. For some, it may be finding a stable place to live. For others, it could be something as simple as getting a ride to an appointment. Whatever your struggle, we are here to support you through a wide range of services designed to help you stay healthy and thrive.

If housing is one of your challenges, Health Plus Indiana offers multiple housing programs, including emergency shelter, short-term rental and utility assistance, and permanent housing support for people living with HIV. If you need a safe place to live, we will help you find one—and work with you to maintain it.

We also understand that affording health care and everyday expenses can be overwhelming. Our insurance enrollment specialists are available at every location to help you apply for Medicaid, Medicare, marketplace plans, and patient assistance programs—at no cost to you. If you experience an unexpected financial hardship, our Direct Emergency Financial Assistance program can help cover rent, utilities, medical costs, and other urgent needs.

Staying nourished and making it to your appointments are just as important. Our food and nutrition program provides monthly access to a food pantry, medically tailored meals, vitamins, and fresh produce from community tower gardens. Need a ride to an appointment, schedule with our transportation program. They offer free rides to medical and social service appointments, and can even deliver food and medications directly to your door.
