No matter where you live in Indiana, we encourage you to reach out. Health Plus Indiana serves individuals and families across the state. Some services may be limited based on the funding source that supports them — see the FQHC и Ryan White Part C tabs for the specific areas each program covers.

Federally Qualified Health Center (Look-Alike) service area
As a Federally Qualified Health Center Look-Alike, Health Plus Indiana provides primary and preventive health care in two parts of Indiana:
- North: St. Joseph and Elkhart Counties
- South: Scott County, including Austin and Scottsburg

Ryan White Part C (HIV care) service area
