This means the amount you pay is based on your income and family size. We compare your household income to the <strong>Federal Poverty Level (FPL)<\/strong>. The FPL is a number set by the government each year. It helps programs like ours figure out how much help you need.<\/p>\n<p>Here is how our fee scale works:<\/p>\n<ul>\n<li><strong>0 to 100% FPL<\/strong> \u2014 Free care (100% discount)<\/li>\n<li><strong>101 to 150% FPL<\/strong> \u2014 90% discount<\/li>\n<li><strong>151 to 200% FPL<\/strong> \u2014 80% discount<\/li>\n<li><strong>201 to 250% FPL<\/strong> \u2014 70% discount<\/li>\n<li><strong>250 to 300% FPL<\/strong> \u2014 60% discount<\/li>\n<li><strong>300 to 350% FPL<\/strong> \u2014 50% discount<\/li>\n<\/ul>\n<p>Even if you do not qualify for a full discount, you may still save a lot on your care.<\/p>\n<h2>What You Need to Apply<\/h2>\n<p>To find out your discount level, please bring these items to your visit:<\/p>\n<ul>\n<li><strong>Proof of income<\/strong> \u2014 such as a pay stub, tax return, or benefits letter<\/li>\n<li><strong>Photo ID for all household members<\/strong> \u2014 such as a driver&#8217;s license, state ID, or passport<\/li>\n<li><strong>Proof of residency<\/strong> \u2014 such as a utility bill, lease agreement, or bank statement with your address<\/li>\n<\/ul>\n<p>You can fill out an application at the front desk of any Health Plus Indiana location. You can also apply online. Our staff will walk you through every step if you need help.<\/p>\n<h2>How Often Do I Need to Reapply?<\/h2>\n<p>Your fee scale level is reviewed once a year. If your income or family size changes during the year, please let us know.
